Transition Home Healthcare is an elite home health agency serving Houston, TX and surrounding areas since 2010. We are a group of passionate people dedicated to delivering the highest quality services to our patients, families, team members, and the community. Our team is growing and looking for a dedicated: Occupational Therapist (OT) Shifts Available: Full Time, Part Time & PRN Service Areas: Northeast Houston, Lake Jackson, Conroe, Northwest Houston, Southwest Houston, Missouri City, Angleton Responsibilities:

• Facilitate rehabilitation of patients with emotional, physical, or mental disabilities.
• Develop and implement occupational therapy care plans.
• Coordinate care with home healthcare team.
• Evaluate therapy needs and monitor and report patient progress and outcomes.
• Educate patients and family members.
• Prepare timely, complete, and accurate clinical notes.
• Communicate effectively with patients, caregivers, healthcare providers, office support, and other care team members.

Qualifications:

• Occupational therapist in the state of Texas.
• Home health experience highly preferred.
• Knowledge of home health regulations and general healthcare compliance.
• Able to handle physical workload.
• Good driving record.
• Experience with HCHB EMR (preferred but not required).
• Effective communication and people skills.
